SAMPLE FORM To request your own site, please complete the following form. It usually takes only a few minutes for your website to be created and you will receive an email once it's done. The email will contain the website address of your Account Admin Site where you can finalize specific information for your site (ie. providing text to customize your home page, setting your tax rate, tax state and shipping charges, specifying the products you wish to sell, etc.). Your site will not be available to the public until you complete these simple configuration steps. This address will be used to communicate with you about your site, for monthly invoices, etc. You will be able to specify a different email address (Site Email Address) on your Account Admin Site where your orders and emails from your website will be sent, if you so desire. Owner's Email Address*:         UserID & Password Please create a unique UserID and Password below. These will be required to access your Account Admin Site. If you enter a UserID that has already been used, you'll be prompted to selected a different UserID. The directory name will be part of the website address people will enter to access your site. This name must be unique for each site hosted on our service. When you click the "Submit" button below, the system will check to be sure the name you entered has not already been used. If it has, you will be asked to enter a different name.   Only letters, numbers, the dash character "-", and the underline character "_" are allowed; no blank spaces. We caution you on the usage of the underline character, since it can hard to see when the address is underline. Please keep in mind that this name will be how people will access your site, so choose something that is easy to remember and to type. Also, the name is case sensitive so if you specify "MySite" as your directory name, people will have to enter it exactly that way or they won't be able to access it ("mysite" won't work). Requested web site address*:     www.pacshop.com/   Terms of Service By clicking the "Submit" button below and requesting that a site be created for your use, you agree to the following Terms of Service: All accounts will be billed monthly for service in advance and invoices will be sent via email. Partial month fees will be prorated and included in the first invoice. All invoices are due and payable upon receipt unless prior arrangements have been made. All returned checks will be subject to a $20.00 fee for each time they are rejected by your bank. Account access may be interrupted if payment is over 30 days past due. All prices are subject to change. You will be notified 30 days in advance of any price changes or changes to this Agreement. You have the right to cancel your account at any time, for any reason. An email notice is required and should be sent to support@pacshop.com. Your account will be cancelled at the end of the current month. You are responsible for any fees due through the end of that month. No refunds will be given. We reserve the right to delete any data records associated with your account when your account is closed. PACshop cannot be held responsible for any damages your business may suffer. PACshop makes no warranties of any kind, expressed or implied for services we provide. PACshop disclaims any warranty or merchantability or fitness for a particular purpose. This includes loss of data resulting from delays, and any and all service interruptions caused either by PACshop and its employees or from service interruptions out of our control. PACshop values your privacy. We will never sell, rent, giveaway or share personally identifiable information about you or your business to any third party without first receiving your permission. It is our goal that your site be available 100% of the time but due to hardware maintenance requirements, upgrades, and various forces of nature this might not always be possible. Occasionally system maintenance is performed that could result in your site being unavailable for a short period of time. We will send out an email several days in advance of all planned maintenance so you will know if your site is scheduled to be down. Otherwise, if you find that your site is down, please send an email to support@pacshop.com. You assume all responsibility for advertisement of your site. We will provided information that gives details on submitting your site to the various search engines on the internet so people can find your site. You agree to respect your customer's right to privacy and will not send them advertising emails through our system if they elect not to receive them. PACshop uses secure server software to protect your customer's personal information (credit card number, name, and address) as it is transmitted across the internet. The security of this information is entirely your responsibility once it is accessed from our system. You are responsible for shipping all merchandise ordered from your website. It is your responsiblility to make sure you've received payment before merchandise is shipped. You must be able to process credit card transactions if you indicate that you accept credit cards for payment. PACshop is not responsible for the delivery or quality of any goods or services sold or advertised through a PACshop customer's website. You are solely responsible for the proper conduct of your business and all other matters under your control. PACshop, our affiliates, service providers, or any other party involved in creating, producing or delivering the PACshop service, technology that powers the solution or content available will not be liable to you in any manner, regardless of the type of claim or the nature of the cause of action. We reserve the right to suspend and/or cancel any account in accordance with the provisions of this Terms of Service.
